Terms and Conditions for Health Plus Homecare

1. Introduction

Welcome to Health Plus Homecare. By accessing our website and utilizing our services, you agree to comply with and be bound by the following Terms and Conditions. Please review them carefully. 

2. Services Provided

Health Plus Homecare offers a range of in-home healthcare services, including but not limited to:

• Skilled nursing care

• ICU setups at home

• Physiotherapy sessions

• Medical device installations

• Doctor visits at home

• Ambulance support

• Elderly care services

All services are tailored to meet individual patient needs as prescribed by their healthcare provider.

3. Service Agreement

• Consent: Prior to the commencement of any service, patients or their authorized representatives are required to provide written consent, acknowledging the scope and nature of the services to be provided.

• Care Plan: Services will be delivered in accordance with a personalized care plan developed in collaboration with the patient’s healthcare provider. Any modifications to the care plan should be communicated promptly to Health Plus Homecare.

• Patient Responsibilities: Patients are expected to provide a safe and respectful environment for our healthcare professionals. Any concerns regarding the services should be reported immediately to our support team.

4. Payment Terms

• Billing: Patients will receive detailed invoices outlining the services provided. Payment is due upon receipt of the invoice unless otherwise agreed upon.

• Advance Payments: All services require advance payments or deposits. These will be communicated prior to the initiation of services.

• Late Payments: Failure to make timely payments may result in the suspension or termination of services. Health Plus Homecare reserves the right to charge interest on overdue amounts as permitted by law.

5. Cancellation and Rescheduling

• Notice: Patients wishing to cancel or reschedule a service appointment are required to provide at least 24 hours’ notice. Cancellations made with less than 24 hours’ notice may incur a cancellation fee.

• Provider Cancellations: In the rare event that Health Plus Homecare needs to cancel or reschedule a service, we will provide as much notice as possible and endeavor to arrange a suitable alternative.

6. Medical Equipment

• Provision and Use: Any medical equipment provided by Health Plus Homecare is to be used solely for the intended patient and purpose. Patients are responsible for the proper care and safeguarding of the equipment.

• Return of Equipment: Upon completion of the service or termination of the agreement, all equipment must be returned in good condition. Charges may apply for damaged or unreturned equipment.

7. Confidentiality and Privacy

Health Plus Homecare is committed to protecting the privacy and confidentiality of our patients. All personal and medical information will be handled in accordance with applicable privacy laws and our Privacy Policy.

8. Limitation of Liability

While Health Plus Homecare strives to provide high-quality services, we cannot guarantee specific outcomes. Our liability is limited to the extent permitted by law. We are not liable for any indirect, incidental, or consequential damages arising from the use of our services.

9. Amendments to Terms

Health Plus Homecare reserves the right to modify these Terms and Conditions at any time. Any changes will be communicated through our website. Continued use of our services constitutes acceptance of the revised Terms.

10. Governing Law

These Terms and Conditions are governed by the laws of India. Any disputes arising out of or in connection with these Terms shall be subject to the exclusive jurisdiction of the courts in Kolkata, West Bengal.
